4) Nondiscrimination & Accessibility (Section 1557) — /legal/nondiscrimination-accessibility

Sanova Medical Group does not discriminate on the basis of race, color, national origin, sex (including pregnancy, sexual orientation, gender identity), age, or disability in its health programs and activities. We provide:

5) Telehealth Consent (Arizona) — /legal/telehealth-consent

Arizona law generally requires verbal or written informed consent before delivering care via telehealth, with documentation in the medical record; certain exceptions apply (e.g., emergencies). Prescribing must follow state and federal law. FindLaw Codes+2Arizona Legislature+2

By consenting to telehealth with Sanova Medical Group, you acknowledge:

  1. Nature of telehealth: audio, video, and/or data transmission; potential benefits/risks (e.g., privacy limits, tech failures).

  2. Alternatives: in-person visit is available when clinically appropriate.

  3. Privacy: we protect PHI per HIPAA; you should choose a private location and secure device/network.

  4. Medical emergencies: call 911; telehealth isn’t for emergencies.

  5. Consent: you may withdraw consent at any time by informing us.

  6. Location & licensure: you must be in a state where our clinicians are authorized to practice (typically Arizona for Sanova).

  7. Prescriptions: subject to applicable law; Schedule II substances may require in-person assessment consistent with law. Arizona Legislature+1

6) No Surprises / Good Faith Estimate — /legal/no-surprises

If you’re uninsured or not using insurance, you have the right to a Good Faith Estimate of expected charges before services. If you are billed $400+ more than your estimate, you may dispute the bill.
